Mix plan for Rewards plugin

Please add Mix plan for Rewards plugin.

The rewards plugin already counts ppd + pps.

I mean affilate plans for rewards plugin.
So users need choice one of the plans.
Example:
Plan A: PPS 75%
Plan B: PPD 50€
Plan B: MIXED 25% PPD + 25% PPS

Also please add apply funtion for rewards plugin.
User must apply to rewards and admin need aprove.
Otherwise there lot of unused rewards logs for users thay dont interested with rewards.

Let me explain more..

There must be 3 option for earning plans:
Option 1 PPD:
If users choice this option, they can earn only for PPD with high PPD offers.

Option 2 PPS:
If users choice this option, they can earn only for PPS with high commission offer.

Option 3 Mixed - PPD and PPS:
If users choice this option, they can earn for both earning plans but with low offer.

Also admin must be able to approve or reject who will apply for rewards.
Because rewards working for all users also for not interested users due that there so big unnecessary data in database.

I hope it's understandable and it must be asap done for next version.
